Welcome to Southpoint Rehabilitation and Healthcare Center
Southpoint Rehabilitation and Healthcare Center is a skilled nursing facility serving the greater Durham and Raleigh areas, with capacity for 140 residents. We provide short-term rehabilitation and long-term skilled nursing care, backed by an in-house Therapy Team focused on helping residents regain independence. As part of YAD Healthcare, we deliver personalized treatment plans tailored to each individual’s needs.

Southpoint works closely with leading hospitals, including Duke and UNC, and our approach centers on physical, mental, and social well-being. Ongoing staff education and highly qualified professionals mean residents get consistent, attentive care throughout their recovery.

LONG TERM CARE​

Making the decision to move to long-term care can be difficult, but our skilled nursing facility in Durham is here to make the transition as smooth as possible. Our skilled nursing staff is available 24/7 to provide the highest level of personalized care for each resident. We are dedicated to helping every individual thrive and live their best life.
